Case Manager

L.A. Family Housing is seeking a Case Manager. The Case Manager is responsible for providing case management services to homeless individuals at the Valley Shelter programs. Services include: conducting intake and initial assessment interviews; assisting clients in developing case management plans; making referrals to agency and community-based services as appropriate; and meeting weekly with assigned clients to measure progress in meeting goals and solving problems as needed.
