The Lamp Community Advocate(s) will work with members (caseload of 12-24) to coordinate mental health and recovery services and help them to develop a plan that will assist them in maintaining improved physical and mental health. Candidate must be computer literate, detail oriented, flexible and well organized.
Responsibilities:
Advocate will coordinate and implement mental health and recovery support services with individual members. Ideal candidate must be able to develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health. Candidate will assist in the screening, assessment and enrollment of Village members including orientation to program policies, resources and goals. Develop a complete member file with all required documents that is essential to maintaining up to date, accurate, and complete data. Candidate will assist in involving members in daily activities and scheduled appointments.
Requirements:
Ideal candidate will be detail-oriented, computer literate and able to perform duties on a tight schedule. Must have great organizational, oral and written skills. Candidate must be able to effectively interact with a wide variety of people. Must have insured vehicle with valid California driver’s license. The ability to properly handle confidential information and records is essential. Must have or maintain a current, thorough knowledge of community resources and utilize these to provide comprehensive, wrap-around services to members. Candidate should have some College (Degree preferred) and experience working with homeless adults preferably those struggling with mental illness and/or substance abuse
